The two levels of Heresy, as well as the Prime Sanctum for Act II
Heresy is the sixth layer of Hell and the first of four that make up the City of Dis, as well as the final layer of Act II. This layer is reserved for souls who who practiced and spread false beliefs and religions, spending their eternity in boiling blood or burning from inside coffins that are scattered throughout upside down blazing metropolis, which glows up the glass windows of the giant buildings. Beneath the city resides a massive prison complex, where the Prime Soul of Sisyphus is incarcerated inside the Flesh Panopticon, which is overseeing his helpless followers while the fallen warlord slumbers from within, although its prisoner may tear itself free at any moment should the Panopticon be disturbed.
Similar to Gluttony, Heresy only has 2 levels, one of which houses the entrance to the second Prime Sanctum. This layer introduced Swordsmachine as a common enemy, as well as bringing back both the Insurrectionist and Hideous Mass as boss fights. However, the most important parts of this layer are the rematch with Gabriel in 6-2: AESTHETICS OF HATE, and the battle against the Flesh Panopticon and Sisyphus Prime in P-2: WAIT OF THE WORLD.

- This is the first layer since its introduction to not include the Drone.
- This is the first layer to feature conventionally Satanic imagery, in contrast to the otherwise non-standard depictions of Hell in the rest of the game.
- The music in this layer uses many leitmotifs from earlier in the game:
- "Altars of Apostasy" includes a reprise of "Glory".
- If you listen closely at the last part of the "Glory" leitmotif part, there is a secondary guitar riff that plays a tune similar to the chorus of "Young Girl A" (in a different key and tempo) by the late siinamota.
- "Hall of Sacreligious Remains" is a black metal style version of the "Castle Vein" motif.
- "The Death of God's Will" is a maximalist development of Gabriel's previous theme, "Divine Intervention".
- "PANDEMONIUM" shares leitmotifs with its predecessor theme, "CHAOS".
- "WAR" is structured similarly to "ORDER" and features the "Sands of Tide" motif at its climax.
- "Altars of Apostasy" includes a reprise of "Glory".
